Job Description

We are seeking a compassionate, organized, and energetic Facility Rehab Lead to serve as our on-site representative within one of our partnered senior living communities. In this highly visible role, you'll be the bridge between residents, families, community staff, and our Home Health team—helping coordinate care, promote wellness, and ensure an exceptional resident experience. What You'll Do As the Facility Rehab Lead, you will: Coordinate scheduling of skilled nursing and therapy visits for residents. Attend daily interdisciplinary stand-up meetings to provide updates on home health patients. Communicate changes in residents' medical status promptly to the appropriate clinical team. Lead and oversee non-clinical wellness programs, including: Instructor-led fitness and exercise classes Resident gym supervision Balance screenings and assistive device clinics Wellness education workshops Dementia Care Program coordination Deliver outstanding customer service to residents, families, and community partners. Participate in prospective resident tours and provide clinical support when appropriate. Educate residents and families about home health services and appropriate levels of care. Guide new residents and families through the home health admission process. Perform move-in safety assessments to support successful transitions. Maintain therapy equipment and wellness areas to ensure a safe, functional environment. Represent UT Health at Home with professionalism while promoting our mission, values, and culture. Qualifications Education High School Diploma or GED required Experience Six months to one year of related healthcare, senior living, rehabilitation, or customer service experience required Knowledge of medical terminology preferred Requirements Excellent communication, organizational, and interpersonal skills Ability to work independently and collaboratively with interdisciplinary teams Reliable transportation Valid driver's license and current automobile liability insurance
