Job Description

Job Overview We are seeking a dynamic and compassionate Health and Wellness Director to lead our comprehensive health services within our community-focused facility. This pivotal role involves overseeing all aspects of resident health management, ensuring the delivery of high-quality, person-centered care that promotes well-being, safety, and independence. The ideal candidate will bring energetic leadership, expert knowledge in various healthcare settings, and a passion for enhancing residents' quality of life through innovative care strategies. Duties Develop and implement personalized care plans tailored to each resident's unique needs, including those requiring critical care, hospice, or home health services. Supervise nursing staff and care teams, providing mentorship and ensuring adherence to best practices in nursing management and shift management. Oversee medical records documentation review, ensuring compliance with HIPAA regulations and accurate record-keeping using Electronic Medical Records (EMR) systems. Monitor patient health status through regular assessments, patient monitoring techniques, and physiological evaluations rooted in anatomy and physiology knowledge. Coordinate case management efforts to optimize resident outcomes, including Medicare billing processes and regulatory compliance. Lead interdisciplinary team meetings to review care plans, update progress notes, and ensure seamless communication across departments. Manage budgets related to healthcare services, supplies, and staffing to maintain operational efficiency while delivering exceptional resident care. Ensure all staff are trained in medical terminology, documentation standards, and HIPAA privacy rules to uphold confidentiality and legal compliance. Facilitate ongoing staff training on acute care procedures, emergency response protocols, EMR systems usage, and resident safety measures. Collaborate with residents' families and healthcare providers to foster transparent communication regarding care plans and progress. Experience Proven experience in healthcare management within assisted living, home health, hospice care or acute care environments. Demonstrated supervising experience with nursing teams or healthcare staff in fast-paced settings requiring leadership excellence. Strong background in medical terminology, anatomy, physiology, and patient monitoring techniques essential for high-quality care delivery. Familiarity with EMR systems for documentation review and medical records management is required. Knowledge of Medicare regulations and billing procedures related to resident reimbursement processes. Experience managing budgets within healthcare operations while maintaining compliance with HIPAA standards. Exceptional leadership skills with the ability to motivate teams, foster collaboration, and drive continuous improvement initiatives.
