Patient Service Coordinator- Hospice | FullTime Clerical Support / Administrative Hospice

The Patient Service Coordinator collaborates with the patients, caregivers, referral sources, and interdisciplinary team members to support care coordination and continuity of care. The Patient Service Coordinator is a resource person for all patient scheduling needs and works closely with other members of the team to ensure adequate coordination and transition occurs to other department services. The Patient Service Coordinator should have extensive knowledge of the guidelines governing the department and offered service lines. Manages and tracks supply requisitioning and the stock of supplies needed. Update the clinical on call schedule. Maintain and update GUIDE care navigator contact frequency and respite delivery schedule. Assists with administrative duties as assigned. Attends and actively participates in interdisciplinary team meetings.Required Education:
High school diploma or equivalentPreferred Education:
Some college or business coursesRequired Skills:
Nursing skills as defined in Nursing Standards of practice. Basic computer skills.Required Licensure/Registration/Certification:
Current Texas Driver's License; Valid Auto Liability Insurance. Must have reliable transportation.Preferred Licensure/Registration/Certification:
Advanced education or certification in area of specialtyRequired Experience:
Experience in service coordinationPreferred Experience:
