Are you able to read people and situations effectively? Are you valuable in your ability to influence people to accept change or new ideas? We are looking for an Clinical Manager Assistant - LVN who is intuitive and can take on difficult situations with a positive outcome. You will need to be able to handle a variety of assignments at one time with equal sense of urgency. This position requires someone who is detail oriented, self-sufficient and able to follow through.
Responsibilities and Duties:
Assists RN Clinical Manager/Executive Director in the hiring process including clinical skill and documentation education for new clinical employees as appropriate. Provides assistance as necessary for field clinicians. Returns calls from physician offices, labs or patients who have contacted the home health office and provides back up for clinicians when clinician is making inhome visits. Orders patient nonroutine supplies as requested by field staff and/or the clinical manager team. Review of clinician communication notes and follow up of appropriate interventions. Ensures infection control and occurrence reports process is maintained and appropriate follow up occurs. Receives and follows up on the DME requests, ensuring patient DME needs are met. Assists Clinical Manager to ensure facetoface process/documentation has been received by Day 30 of the initial episode. Assists Clinical Manager with case conference meetings/reviews as requested. Facilitates appropriate referral based upon patient needs to services such as: MOW, DADS, Hospice, other postacute providers. Receives incoming clinical telephone calls and initiates appropriate interventions as required. Escalates clinical needs as appropriate. Assist in identifying clinical educational needs. Assists Clinical Manager in identification and coordination of potential telemonitor patients. Process referrals; communicate with referral sources and sales team. Schedule patients with field clinicians. Performs other duties and activities as delegated by Clinical Manager.
Qualifications and Skills:
Current Texas LVN License Current Certification in BSL/CPR Valid Texas Driver's License and Automobile Insurance Previous experience in home care setting with two years management or supervisory experience and a proven ability to work within an interdisciplinary setting Positive attitude Ability to work as a team member Demonstrates high energy level with capability of multitasking and selfdirection Has excellent observation, good nursing judgement and communication skills Demonstrates excellent verbal and written communication skills Knowledge of WellSky EMR software is a plus Able to work Monday through Friday schedule
