About Us:
At Ageless Simplicity Home Health, we are committed to providing compassionate, personalized care to patients in the comfort of their homes. We are a growing home health agency seeking a dedicated Patient Care Coordinator to join our team and support the delivery of high-quality care by facilitating communication, coordinating services, and ensuring patient satisfaction.
Position Summary:
The Patient Care Coordinator serves as a liaison between the clinical team, patients, families, and referring providers. This role ensures that patients receive timely and appropriate home health services by managing referrals, verifying insurance, coordinating care schedules, and supporting clinical staff with documentation and communication.
Key Responsibilities:
Coordinate and schedule initial evaluations and follow-up visits with clinicians Communicate with referral sources, patients, and families to initiate services Verify insurance benefits and obtain authorizations for services Ensure timely documentation and tracking of patient visits and care plans Follow up on outstanding tasks and communicate progress with team members Maintain accurate records in EMR system and patient logs Serve as a point of contact for patient concerns and escalate as necessary Track admissions and ensure all required intake documentation is complete Monitor patient outcomes and assist with quality assurance processes
Qualifications:
High school diploma or equivalent (Associate's or Bachelor's degree preferred) Experience in home health, healthcare scheduling, or medical office setting preferred Knowledge of EMR systems (e.g., WellSky, Kinnser) and Microsoft Office Suite Excellent communication and organizational skills Ability to multitask, prioritize responsibilities, and follow up on outstanding items Strong customer service and problem-solving abilities Knowledge of home health regulations, Medicare/Medicaid authorizations, and insurance verification is a plus

Temporary Pay:
$18.00 - $22.00 per hour
