Job Description

Looking to join a great team and make a difference in your community? JFS is looking for a motivated Care Manager to provide high-quality services to adults and families. The Care Manager is an integral member of the Care Management Department, responsible for assessing client needs, developing and implementing service plans, and coordinating comprehensive care for older adults and adults with disabilities. This role ensures clients receive services that support their health, safety, independence, and overall wellbeing.
Duties and Responsibilities:
The Care Manager's responsibilities include but are not limited to the following: Conduct client assessments and develop individualized care plans that support health, safety, independence, and overall well-being Coordinate and monitor services, including healthcare, housing, home care, transportation, and community-based resources Provide ongoing support, advocacy, and guidance to clients and families navigating complex healthcare, social service, and benefit systems Monitor client progress, complete reassessments, and adjust service plans to address changing needs and goals Collaborate with healthcare providers, community partners, fiduciaries, and internal team members to ensure coordinated, person-centered care Maintain accurate documentation, manage assigned caseloads, and ensure compliance with agency policies and regulatory requirements Support intake, referral, and eligibility review processes to ensure clients receive appropriate services Participate in on-call coverage and respond to urgent client needs as required Build and maintain relationships with community resources and referral partners to enhance service options for clients Contribute to program initiatives, team collaboration, community outreach, and agency meetings and training activities
Qualifications:
Bachelor's in Social Work or other Human Services degree required, Master's in Social Work preferred Familiarity with providing comprehensive case management with older adults Familiarity with age-related health issues, dementia, grief and loss and multidimensional caregiving Personal skills needed to be successful as a Care Manager are creativity, flexibility, organizational skills, time management and the ability to work both independently and as a team player. A Care Manager must have strong communication skills and be able to engage with the client and their family. The ideal candidate will have experience working with seniors, and the ability to effectively communicate and serve as a liaison between the client and the families served and community agencies.
