Job Description

Overview:
The Patient Services Coordinator is responsible for organizing and maintaining patient care schedules in coordination with field staff, caregivers, and clinical leadership. This role ensures continuity of care by managing shift coverage, verifying care plans, and documenting updates in Axxess. It requires strong communication, problem-solving, and time-management skills, along with a working knowledge of healthcare scheduling systems and practices.
Responsibilities:
Schedule new and existing patients based on care plans, MD orders, and staffing availability. Monitor and confirm scheduling accuracy and authorized visit frequencies in Axxess. Coordinate unassigned shifts by collaborating with Home Health Aide Coordinators (HHAC) and other scheduling resources. Maintain caregiver availability and assignment records in Axxess. Communicate with caregivers to confirm availability, fill open shifts, and provide case orientation. Partner with recruiters and regional teams to maintain a reliable PRN pool and active caregiver roster. Track and follow up on missed or not-started visits through QA and clinical workflows. Manage daily needs list and document all shift-related communication. Support 24/7 staffing coverage in conjunction with the Travel Nurse Program Manager. Maintain accurate records, uphold HIPAA standards, and safeguard patient information. Perform other duties as assigned.
Qualifications:
High school diploma or equivalent required. Prior experience in healthcare scheduling or coordination strongly preferred. Proficiency with Axxess or other EMR/scheduling platforms. Excellent communication and interpersonal skills. Strong attention to detail and organizational skills. Ability to manage multiple priorities in a fast-paced environment. Professional demeanor and appearance. Must pass a criminal background check and drug screen, as required by applicable law.
Pay Range:
USD $43,000.00 - USD $47,500.00 /Yr.
