JOB SUMMARY

The Care Management Supervisor provides leadership, oversight, and support for care management services within the MSSP program. Reporting to the Program Director, this position is responsible for supervising staff, monitoring program performance, improving service delivery, and ensuring participants receive high-quality, person-centered care. The supervisor works closely with internal teams, healthcare providers, community organizations, academic partners, and state agencies to ensure program excellence and regulatory compliance.

ESSENTIAL FUNCTIONS

Team Leadership & Staff Development Supervise and support Care Managers, Nurse Care Managers, Community Health Workers, Care Manager Aides, interns, volunteers, and support staff. Provide coaching, mentoring, performance evaluations, and corrective action plans when needed. Assign and monitor caseloads to ensure equitable workload distribution and quality service delivery. Facilitate staff meetings and promote a positive, collaborative team environment. Coordinate and deliver ongoing staff training in areas including documentation, HIPAA, safety, quality improvement, and program policies Program Operations & Quality Improvement Monitor program performance, service delivery outcomes, and key performance indicators. Review participant records, contracts, encounters, and program documentation for completeness and compliance. Identify opportunities to improve processes, access to services, and participant outcomes. Recommend and implement updates to policies, procedures, and operational practices. Provide regular program reports and performance updates to leadership. Community Engagement & Partnership Building Develop and maintain strong relationships with healthcare providers, community organizations, educational institutions, and government agencies. Represent California Health Collaborative at community meetings, provider forums, state hearings, and professional events. Support outreach initiatives that increase awareness of available services and resources. Foster collaboration across MSSP, Enhanced Care Management (ECM), and Community Health Worker (CHW) programs. Internship & Workforce Development Partner with university field liaisons and internship supervisors to support student learning experiences. Provide guidance, coaching, and weekly supervision for interns. Help cultivate the next generation of health and social service professionals. Cross-Program Collaboration Collaboration across CHC programs to increase awareness of CHC services, promote CHC-branded programs and standardized referral pathways, and consistently utilize CHC's online referral asset (including the CHW Navigation hub and related support programs) to strengthen access, coordination, and continuity of care. External Awareness & Representation Proactively promote CHC priorities, programs, and referral assets to all external stakeholders—including enrollees, partner organizations, and community entities—to eliminate fragmentation, prevent missed referral opportunities, and ensure consistent awareness and utilization of CHC services.

REQUIRED EDUCTION AND/OR EXPERIENCE

Master's degree in public health, Community Health, Social Work, Nursing, Gerontology, Healthcare Administration, or a related field. OR Bachelor's degree in one of the above fields plus four years of relevant professional experience. Minimum three years of leadership, management, or administrative experience. Demonstrated ability to supervise and motivate interdisciplinary teams. Strong organizational, problem-solving, and decision-making skills. Excellent written, verbal, and interpersonal communication skills. Proficiency with Microsoft Office Suite, Outlook, SharePoint, and electronic documentation systems. Commitment to serving diverse communities and advancing health equity. Preferred Qualifications Certified Case Manager (CCM) Licensed Clinical Social Worker (LCSW) Licensed Marriage and Family Therapist (LMFT) Experience in care management, aging services, public health, healthcare, or social services. Experience supervising multidisciplinary teams. Language Skills Fluent in English with good communication and writing skills, bilingual preferred. Travel Requirements Reliable transportation and valid automobile insurance required. Must be willing and able to travel to all service areas including Alpine, Colusa, El Dorado, Placer, Sacramento, Sutter, Yolo, and Yuba counties.
