Job Description

Are you passionate about helping patients navigate the healthcare system and ensuring they receive the care they need? Join a mission-driven healthcare organization where your work directly impacts patient access to specialty care, diagnostic services, and community resources. As a Referral Coordinator, you will serve as a key point of contact for patients, providers, specialists, and insurance carriers, ensuring referrals are processed accurately, efficiently, and in a timely manner.
What You'll Do:
Process, track, and coordinate patient referrals and diagnostic orders Verify insurance coverage, benefits, and authorization requirements Schedule specialty appointments and coordinate care with outside providers Maintain accurate patient records and documentation Communicate with patients, specialists, health plans, and community organizations Serve as a resource for patients by helping navigate healthcare, financial, and social barriers Ensure timely follow-up and resolution of referral requests Support care coordination and continuity of care efforts
What We're Looking For:
High School Diploma or equivalent 2+ years of healthcare, referral coordination, medical office, patient services, or related experience preferred Medical Assistant training or certification preferred Knowledge of insurance plans, authorizations, and healthcare terminology Experience with Electronic Health Records (NextGen preferred) Strong customer service, communication, and problem-solving skills Exceptional attention to detail and organizational abilities Ability to thrive in a fast-paced healthcare environment while maintaining patient confidentiality Job Type & Location This is a Permanent position based out of Sacramento, CA. Pay and Benefits The pay range for this position is $50000.00
