Job Description

Job Description Everest Health Care Solutions is seeking a full-time Care Coordinator (CC) for our community-based program. The Care Coordinator will be responsible for overseeing staffing operations, coordinating care services, maintaining communication with clients and families, and ensuring overall quality of care delivery in the field. The Care Coordinator will assist with day-to-day operational management, including supervising staff coordination, ensuring required supervisory visits and case conferences are completed, and reviewing client-specific plans of care with field staff. This is a full-time, in-office position with up to 10% travel within the assigned county to complete supervisory visits when required. This role offers excellent professional experience for individuals with backgrounds in healthcare administration, social work, public health, psychology, human services, or business management. Key Responsibilities Coordinate medical and non-medical care programs Serve as a communication liaison between State Support Coordinators, staff, patients, and families Manage patient case coordination and ongoing follow-ups Recruit, onboard, and train field staff Create, review, and approve staff schedules Match referrals with available or newly hired staff Ensure completion of background checks and new hire documentation Coordinate meet-and-greet visits between families, clients, and field staff Conduct quality assurance follow-ups through surveys and regular family communication Prepare and submit incident reports as required Maintain patient confidentiality and uphold quality care standards Attend required internal meetings and operational reviews Qualifications Experience in home care, healthcare coordination, or medical terminology preferred (equivalent education and experience may be considered) - Degree required Ability to work effectively in a fast-paced, dynamic environment Strong organizational skills with the ability to manage multiple responsibilities simultaneously Excellent written and verbal communication skills with all levels of internal and external stakeholders Ability to exercise sound judgment and maintain professional relationships Strong problem-solving and coordination abilities Proficiency in Microsoft Office applications including Outlook, Excel, and Word Experience working collaboratively to drive action plans and operational outcomes Fluency in English required; Spanish fluency is a plus Education & Experience Requirements Degree required in Human Services, Public Health, Social Work, Healthcare/Business, or related science field Human Resources, recruiting, staffing coordination, or healthcare administration experience preferred Customer service experience preferred
