Job Description

The Population Health & Outcomes Manager leads chronic disease care coordination, clinical integration, population health performance, partnership development, workflow improvement, and outcome measurement for approved Rural Health Transformation Program (RHTP) activities. This position supports the implementation and evaluation of initiatives designed to improve patient outcomes, strengthen care coordination, enhance clinical workflows, and advance population health across Adams Health. This is a grant-funded, time-limited position dependent on continued
RHTP/GROW
grant funding and organizational need. Future funding is not guaranteed. Eligible for medical coverage on your 1st day of employment, all other benefits will be effective the 1st of the month following hire date!
Requirements:
40 hours per week
  • First Shift Monday
  • Friday 8:00 AM
  • 4:30 PM Bachelor's degree in Nursing, Public Health, Healthcare Administration, Population Health, or a related field required.
Master's degree preferred. Active clinical license preferred for clinically accountable duties. Minimum 2 years of healthcare operations, population health, care management, clinical integration, quality improvement, or related leadership experience.
