3 month contract Do you have experience coordinating home care services, managing caregiver schedules, or supporting healthcare operations? We are seeking an experienced In-Home Services Coordinator to oversee the daily coordination and delivery of in-home services for PACE participants. This role is not a general administrative or customer service position — we are looking for someone with hands-on experience in healthcare scheduling, home care coordination, caregiver staffing, or patient service operations. The ideal candidate understands the importance of ensuring the right caregiver is assigned to the right participant, maintaining accurate schedules, communicating with families and care teams, and resolving issues quickly to prevent gaps in care. This is a remote position; however, candidates must have the ability to independently manage a high-volume healthcare coordination workload, communicate effectively with field staff, and prioritize participant needs in a fast-paced environment. Responsibilities Manage and maintain daily schedules for in-home care staff, including caregivers, CNAs, LPNs, and RNs. Ensure participant services are delivered according to individualized care plans, provider orders, and required service hours. Coordinate staffing coverage and quickly address call-offs, schedule changes, and service disruptions. Communicate directly with participants, families/caregivers, field employees, and healthcare teams regarding care schedules and updates. Monitor completion of scheduled visits and follow up on missed or incomplete services. Maintain accurate documentation within electronic health record and scheduling systems. Identify and escalate concerns related to participant care, staffing, or service delivery. Support compliance with healthcare regulations, company policies, and contractual requirements. Participate in team meetings, operational updates, and an on-call rotation as needed. Build strong working relationships with caregivers and internal teams to support quality care delivery. Required Qualifications Minimum 2 years of healthcare, home care, social services, or medical administrative experience. Experience coordinating schedules, staffing, patient services, or caregiver assignments. Experience working with electronic documentation systems and maintaining confidential patient information. Strong organizational skills with the ability to manage multiple priorities and urgent scheduling needs. Ability to communicate professionally with patients, families, caregivers, and healthcare professionals Strongly Preferred Experience Home health, hospice, private duty, PACE, senior care, or community-based healthcare experience Caregiver/CNA/LPN/RN scheduling experience Experience coordinating services for elderly or medically complex populations Experience working with care plans, authorizations, service orders, or visit verification systems Please Note Before Applying This position requires healthcare coordination experience .
