Job Description

Overview We are seeking a dynamic and dedicated PHC Coordinator/Director to lead and oversee our primary health care (PHC) programs. This role is vital in ensuring the delivery of comprehensive, high-quality health services that meet the needs of individuals with developmental disabilities and behavioral health challenges. The ideal candidate will bring energy, strategic vision, and a passion for community health to foster positive outcomes and strengthen relationships within the community. As a key leader, you will coordinate multidisciplinary teams, manage program development, and ensure compliance with all regulatory standards while promoting a culture of compassion, excellence, and continuous improvement. Duties Lead the planning, implementation, and evaluation of primary health care services tailored to individuals with developmental disabilities and behavioral health needs. Supervise and support clinical staff, social workers, and administrative personnel to ensure effective service delivery and team cohesion. Develop and manage program budgets, monitor financial performance, and oversee resource allocation to maximize program impact. Build and maintain strong relationships with community partners, healthcare providers, families, and stakeholders to foster collaboration and enhance service access. Oversee compliance with healthcare regulations, licensing requirements, and organizational policies to ensure quality assurance. Coordinate training initiatives to enhance staff skills in behavioral health, social work practices, and program management. Drive continuous quality improvement efforts by analyzing data, implementing best practices, and adapting programs to meet emerging community needs. Skills Proven management experience leading multidisciplinary teams in healthcare or social service settings. VESTA experience is a must bilingual is a must Strong background in program development, implementation, and evaluation within community health or social services. Extensive experience working with individuals with developmental disabilities and behavioral health conditions. Excellent relationship management skills to foster partnerships with community organizations, healthcare providers, families, and government agencies. Solid administrative expertise including budgeting, reporting, compliance monitoring, and documentation processes. Demonstrated supervisory skills with the ability to motivate staff and promote professional growth. Knowledge of social work principles combined with a passion for improving client outcomes through compassionate care. Join us in making a meaningful difference!
