Job Title:

Health Care Coordinator / Resident Care Coordinator (HCC/RCC)

Job Summary:

The HCC/RCC is responsible for coordinating and overseeing resident care services to ensure compliance with state regulations (WAC) and company policies. This role includes leadership of the care team, on-call responsibilities, occasional floor coverage as a med tech/aide, and active monitoring of resident status and staff performance.

Key Responsibilities:

Lead, train, and support care staff to ensure quality resident care. Oversee medication management and PointClickCare (PCC) documentation. Monitor residents for changes in condition and ensure timely interventions. Ensure compliance with WAC regulations and facility policies. Participate in care plan development and family communication. Cover shifts as needed to maintain safe staffing levels. Serve as part of the on-call rotation for emergencies or staffing needs.

Qualifications:

Experience with medication management and PCC preferred. Strong leadership and communication skills. Ability to work flexible hours and respond to on-call needs. Knowledge of state regulations (WAC) and resident care best practices.

Full-time Pay:

$24.00 - $26.00 per hour
