Job Description
Overview Join our dedicated healthcare team as a Home Health Aide, where your compassionate care and proactive support make a meaningful difference in patients' lives. In this vital role, you will provide personalized assistance to individuals in their homes, promoting independence, safety, and well-being. Your energetic approach and attention to detail will help create a comfortable environment for clients of all ages, especially those with complex health needs or disabilities. This position offers the opportunity to work closely with healthcare professionals and gain valuable experience in home healthcare settings. Duties Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting, and mobility support using proper patient transfer methods and equipment like Hoyer lifts. Monitor patient health by observing vital signs including blood pressure, pulse, respiration, and temperature; report changes promptly to healthcare providers. Document patient care activities thoroughly using Electronic Health Record . Provide emotional support and effective client communication to foster trust and comfort during home visits. Assist with light housekeeping tasks including cleaning, laundry, meal preparation, and ensuring a safe environment for clients with mobility challenges or those receiving hospice or end-of-life care. Qualifications Proven experience working as a Home Health Aide or in assisted living, nursing homes, or long-term care environments. Strong knowledge of medical terminology, patient observation techniques, and basic life support (BLS) including CPR and first aid certification. Familiarity with home health agency operations, home & community care practices, and working within home health regulations. Experience working with geriatric patients, individuals with developmental disabilities or dementia-related conditions such as Alzheimer's disease. Ability to perform heavy lifting safely and operate patient transfer equipment like Hoyer lifts; physically capable of supporting patients during transfers. Excellent communication skills for effective client interaction and collaboration with healthcare teams. Knowledge of HIPAA standards to maintain patient confidentiality at all times. Prior experience using EMR/EHR systems for accurate documentation of patient care activities is preferred. Valid certification in CPR/BLS; additional certifications in hospice or dementia care are a plus. Pay:
