LPN Full Job Description Role Overview:
The Primary Care Patient Care LPN collaborates with providers and the care team to deliver safe, patient-centered care in a primary care setting. They partner with patients to improve health outcomes, adhering to organizational standards and values. The role requires performing clinical duties independently with minimal oversight, demonstrating strong communication, organizational, and interpersonal skills.Qualifications & Skills:
- Graduate of an accredited practical nursing program
- Licensed as an LPN in the practice state
Preferred:
6 months clinical healthcare experience, ideally in primary care- Team-oriented with excellent communication and customer service skills
- Flexible, detail-oriented, and self-directed
Physical Requirements:
- Ability to lift/push 50 pounds regularly
- Prolonged standing and walking
- Capable of gripping, reaching, bending, kneeling, twisting, and squatting
