Job Description

Duties Provide assistance with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor patients' health by taking vital signs, observing changes, and reporting findings accurately while adhering to HIPAA regulations for privacy and confidentiality Support medication administration and care plan implementation under supervision or as trained, ensuring safety and compliance with healthcare standards Assist with meal preparation, cooking nutritious meals tailored to individual dietary needs, and maintaining cleanliness in client living spaces Use specialized equipment such as Hoyer lifts or other assistive devices to safely transfer residents with mobility challenges or disabilities Engage residents in social activities and provide companionship to promote mental well-being and reduce feelings of isolation Maintain detailed documentation of care provided, patient observations, and any incidents or changes in condition to ensure continuity of care Qualifications Prior experience in group homes, assisted living facilities, nursing homes, or home health environments is highly preferred Knowledge of patient observation techniques, dementia care (including Alzheimer's and memory care), autism support, hospice care, and working with individuals with developmental disabilities or disabilities Certifications such as First Aid, CPR, HIPAA compliance training, and relevant nursing or caregiving credentials are advantageous Ability to perform heavy lifting safely using equipment like Hoyer lifts when necessary Strong communication skills for effective interaction with clients, families, and healthcare teams Demonstrated commitment to patient safety through vigilant monitoring of vital signs and overall well-being Flexibility to work various shifts including evenings or weekends as needed Embark on a rewarding career where your compassion fuels positive change.
