- Position Profile
- The hospice social worker provides psychosocial support to patients and families by providing skilled social work intervention to include, but not limited to providing resources, referrals, education, supportive counseling, discharge planning and assistance with final arrangements.
- Responsibilities
- + Develops and maintains therapeutic relationships.
- Minimum Qualifications
- + Bachelor's degree in social work from an accredited school and a minimum of 3-5 years palliative care or hospice experience.
- Preferred Qualifications
- + Master's degree in Social Work from an accredited school and a minimum of one year social work experience in a health care setting (hospital, home health, hospice, behavioral health.
