We are looking for an organized, dependable, and caring professional to join our team as an Assessment Coordinator . In this role, you'll play an important part in ensuring our clients receive the personalized, consistent care they deserve. This position combines meaningful in-home client visits with office coordination, scheduling, documentation, and caregiver support. You'll build relationships with clients and their families, support our caregivers, coordinate care, and help ensure that no client goes without the services they need. What You'll DoClient Assessments & Care Coordination Conduct initial in-home client assessments to understand each client's needs, preferences, and care requirements. Complete required 30-day and 90-day supervisory visits and reassessments. Document client needs and update care plans as needed. Communicate changes in client needs with caregivers and the care team. Build trusting relationships with clients and their families. Provide professional, compassionate support and excellent customer service. Office & Administrative Support Maintain accurate client authorizations, records, and documentation. Coordinate scheduling for new and existing clients. Serve as a friendly and professional point of contact for clients, caregivers, and referral partners. Assist with daily office operations. Keep client and scheduling information accurate, organized, and up to date. Staffing & Client Coverage Help ensure clients receive their scheduled care by coordinating coverage for call-outs and open shifts. Contact caregivers to fill staffing gaps and minimize missed visits. Work closely with management to address urgent staffing needs. Step in as a backup caregiver when needed, providing hands-on, non-medical care to ensure clients receive the support they need. Qualifications High school diploma or GED required. Previous experience in home care, caregiving, healthcare, or a related field preferred. Strong organizational and time-management skills. Excellent communication and interpersonal skills. Ability to multitask and manage competing priorities. Ability to work independently and make sound decisions. Comfortable using computers and willing to learn agency software. Valid driver's license, reliable transportation, and proof of auto insurance required. Willingness to travel throughout the Tucson service areas .
