Minimum Qualifications:

High school or general equivalency diploma required Certification from a recognized home health aide program or an Associate's or Bachelor's degree in health administration, medical assisting, nursing, psychology, or a related field Comfortable providing medical care in close physical proximity to patients Exceptional interpersonal communication skills Empathy for the hardship experienced by the patient and the patient's family Ability to maintain composure with patients who present challenging behavior Home Health experience a plus Must have a CA Driver's License with an automobile that is insured in accordance with state and/or Agency requirements and is in good working order Physical Examination (within the last 1 year) PPD Test/ TB Gold test (within the last year) or Chest X-Ray (within the last 2 years) CPR/BLS Certificate Have satisfactory references from previous (or current) employers and professional peers. Job Expectations and Responsibilities:

Performing personal care needs after given a patient's assignment and attending to his/her requests promptly. Meeting safety needs of patient and using equipment safely and properly (foot stools, side rails, O2) Giving personal care including baths, oral hygiene, shampoos and changing bed linen as often as assigned. Assisting in dressing and undressing patients as assigned. Assisting in feeding the patient as assigned. Taking and recording oral, rectal and axillary temperatures, pulse, respiration and blood pressure when ordered with appropriate completed/demonstrated skills competency. Reporting on patient's condition and significant changes to the Case Manager. Also aware of the caregiver or other individuals living with the patient and interpersonal issues. Assisting in ambulation and exercise as instructed by the hospice nurse or therapist. Performing range of motion and other simple procedures as an extensional therapy service as ordered with appropriate completed/demonstrated skills competency. Keeping patient's living area clean and orderly, as assigned. Adhering to the organization's documentation and care procedures and standards of personal and professional conduct. Respectful of patient and family/caregiver environment and patient's personal needs.
