In this vital role, you will provide personalized, respectful care to enhance comfort, dignity, and quality of life. Your compassionate presence and attentive support will help patients and their families navigate challenging times with empathy and professionalism. This paid position offers an opportunity to grow your skills in hospice care while making a positive impact every day. Duties Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility to promote independence and comfort Monitor and document patient observations, including vital signs, medication administration, and changes in condition using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems Support patients with specialized needs such as Alzheimer's care, dementia care, memory care, and working with individuals with developmental disabilities or disabilities Provide emotional support and companionship to patients, fostering a calming environment during difficult times Assist with medical procedures including catheter care, Hoyer lift transfers, and basic wound care as directed by nursing staff Maintain a clean and safe environment by assisting with light housekeeping tasks like laundry, cleaning, meal preparation, and ensuring infection control protocols are followed in compliance with HIPAA regulations Collaborate closely with nursing staff to ensure comprehensive patient care and accurate medical documentation Requirements Prior experience in hospice care, nursing homes, assisted living facilities, or home health is highly preferred Knowledge of medical terminology and familiarity with EMR/EHR systems for documentation Certification in CPR is required; additional certifications such as medication administration or catheter care are a plus Experience working with seniors or individuals with disabilities or chronic conditions such as Alzheimer's or dementia enhances your effectiveness in this role Ability to lift safely using equipment like a Hoyer lift and perform patient transfers while maintaining safety standards Strong communication skills to effectively observe patient needs and document findings accurately Compassionate attitude with a commitment to providing respectful, patient-centered care Join us in delivering exceptional hospice services where your caring nature transforms lives.
