Duties Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor and document patient vital signs, health status, and progress using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems Administer medications accurately following medical instructions and maintain detailed medical documentation in compliance with HIPAA regulations Support clients with specialized care needs such as catheter care, hospice support, dementia, Alzheimer's, and memory care Operate assistive devices like Hoyer lifts safely to promote mobility and prevent injury Conduct patient observation to identify changes in condition or behavior and report findings promptly to supervising healthcare professionals Perform light housekeeping tasks including cleaning, laundry, meal preparation, and ensuring a safe home environment for clients Assist with medical procedures such as taking vital signs, working with individuals with disabilities or developmental challenges, and supporting hospice or palliative care needs Requirements Valid certification as a Home Health Aide (HHA) or Certified Nursing Assistant (CNA) with current state licensing Proven experience in home health care, assisted living facilities, nursing homes or long-term care environments Familiarity with medical terminology and documentation standards for accurate record keeping Knowledge of EMR/EHR systems for efficient patient data management CPR certification and training in emergency response procedures Experience working with individuals with dementia, Alzheimer's disease or developmental disabilities preferred Ability to work independently while maintaining excellent communication skills and professionalism Compassionate attitude with a strong commitment to patient safety, privacy (HIPAA compliance), and quality care Join us to provide exceptional home health services that empower clients to live comfortably and confidently at home.
