Job Description

Job Overview Provide direct support and supervision to participants throughout the program day. Implement individualized goals and support plans as outlined in each participant's Individual Program Plan (IPP/ISP). Teach independent living skills, communication skills, social skills, and self-advocacy. Lead and participate in community-based activities, volunteer opportunities, recreational outings, and educational experiences. Provide vocational training and coach participants in work-related tasks and job readiness skills. Encourage participant choice, independence, and person-centered decision-making. Assist participants with mobility, transfers, and personal care as needed while maintaining dignity and respect. Ensure participant health and safety by following emergency procedures, reporting incidents, and maintaining appropriate supervision at all times. Complete daily documentation, progress notes, attendance records, and incident reports accurately and on time. Safely transport participants to and from community activities using agency vehicles when assigned. Conduct daily vehicle inspections and maintain cleanliness of agency vehicles. Communicate professionally with families, caregivers, Regional Center staff, and interdisciplinary team members. Follow all agency policies and procedures, Title 17 and Title 22 regulations, HCBS Final Rule requirements, and participant rights. Participate in required staff meetings, and trainings Maintain confidentiality and protect participant information in accordance with HIPAA and agency policies. Foster a positive, respectful, and inclusive environment that promotes community integration and meaningful participation. Perform other duties as assigned by management. Experience Experience working with individuals with intellectual or developmental disabilities preferred. Valid California driver's license with a clean driving record. Ability to pass DOJ/FBI background clearance and all pre-employment requirements. Current CPR/First Aid certification or ability to obtain after hire. Excellent communication, organizational, and interpersonal skills. Ability to work independently and as part of a team. Physical Requirements Ability to stand, walk, bend, reach, and lift up to 25 pounds. Ability to participate in community outings and recreational activities.
