In this vital role, you will provide attentive, respectful assistance to promote independence, safety, and well-being. Whether working in a home health setting, assisted living community, or long-term care facility, you will be at the forefront of delivering high-quality patient-centered care that fosters comfort and dignity. Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support Monitor patients' health status by observing vital signs and reporting any changes to healthcare professionals Support medication administration following prescribed care plans while adhering to HIPAA regulations for patient privacy Prepare nutritious meals and assist with meal planning to meet individual dietary needs Operate patient transfer equipment such as Hoyer lifts and ensure safe manual handling during transfers and repositioning Maintain a clean and safe environment by performing light cleaning duties and ensuring infection control protocols are followed Provide companionship and emotional support to clients, fostering a positive and respectful relationship Assist with transportation for appointments or community outings using appropriate personal transportation methods Document patient observations, care activities, and progress accurately in compliance with healthcare standards Qualifications Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly desirable Knowledge of patient observation techniques, dementia care (including Alzheimer's and memory care), and hospice or palliative care practices Familiarity with care plans, medication administration procedures, vital signs measurement, and patient monitoring techniques Ability to perform heavy lifting and manual handling safely using proper techniques and equipment such as Hoyer lifts Certifications in CPR and First Aid are required; additional certifications in HIPAA compliance or dementia care are a plus Experience working with individuals with developmental disabilities, neurodevelopmental disorders, autism spectrum disorder (ASD), or intellectual disabilities is advantageous Strong communication skills for effective client interaction and team collaboration Valid training in patient transfer methods and experience supporting patients with neurodegenerative conditions or geriatrics Join us in providing compassionate, personalized care that enhances quality of life for those we serve.
