SUPPORT PROFESSIONAL 1
Position Description :
Support adults with developmental disabilities to accomplish daily living activities at the person's home and/or in the local community. Help the individual to be a known and valued member of his/her local community. The attendant is directly responsible for personal care and support to meet the goals and objectives of the individual that they serve. The Direct Support Professional 1 (DSP 1) shall be aware that the consumers may terminate individual service. FUTURE TRANSITIONS, INC., is an At-Will employer, meaning that the employer or employee with or without cause, adhering to the law governing employee/employer relationships, may terminate employment at any time. The DSP will understand that Future Transitions Inc. has an obligation to the clients we serve and due to their vulnerability, the DSP's employment is contingent on a successful completion of a background check, Live Scan and DMV report.
Essential Job Duties:
1. Assist or perform for the consumer(s) in personal care, such as grooming, feeding, toileting, meal preparation. Assist or perform for consumer(s) basic housekeeping tasks, laundry and grocery shopping, and other personal care services as assigned by
DSP 2. 2.
Support and guide the consumer in the choices they make regarding mobility, community access, and learning new skills. 3.
Assist or facilitate:
medical appointments, communication, equipment repair, accessing recreational activities, prescribed exercises, and schedule generic travel arrangements and all activities as assigned by the
DSP 2. 4.
Provide companionship, guidance and supervision during Community activities. 5. Record daily in the consumer logs. This includes, but is not limited too, the following: SLS Consumer Daily Notes, Medication Log. 6. Report immediately any changes in the consumer's mental, physical or emotional condition to supervisor. 7. Ensure consumer's health and safety during assigned hours of responsibility. Ensure that consumer takes self-administered medication. 8. Assist in the overall maintenance of the consumer's home and general upkeep of adaptive equipment as assigned. Report any damages to the property or needed repairs for safety purposes immediately to the Direct Support Professional 2 (DSP 2). Abide by all home rules, property agreements and equipment warranties. 9. Perform all duties as specified by the consumer, the Direct Support Professional 2, and 3 (DSP 2, and 3) and the Area Coordinator as detailed in the ISP and the Daily Task List.
Qualifications and Requirements Education and Experience :
High School Diploma or equivalent is preferred. One year of experience providing care to persons with developmental disabilities. Experience working with seniors in a human service field is also acceptable.
Transportation:
a. Valid California drivers license. b. Driving record for the past 7 years (DMV printout required yearly). c. Current vehicle registration and use of a vehicle in good running condition during shift hours. d. Valid automobile insurance for that vehicle and all passengers, with employee name listed on the policy (new verification required at the expiration of the policy).
Other:
Current CPR and First Aid Certification for adults and children. Cell phone with data plan and app capabilities. Successfully complete a criminal background check through fingerprint clearance. Employment will be contingent upon results. The willingness and ability to perform all the essential functions of the job. The ability to work collaboratively with consumers and co-workers. An employment record of punctuality, good work attendance and reliability. You must complete an I-9 and show valid proof of eligibility to work in the U.S.
Pay Range:
Based on experience - $19.50 - $21.50 hourly
