Full Job Description Job Summary:
Our JCAHO-certified Home Health Agency is seeking compassionate Home Health Aides to provide essential care and support to individuals in their homes. As a Home Health Aide, you will follow the Aide Care Plan determined by the Registered Nurse to assist clients with daily activities, personal care, and ensure their well-being.Duties:
- Carry out Home Health Aide Plan of Care as developed by the RN
- Monitor vital signs by taking and recording temperatures, pulse, respiration and blood pressure when ordered and report any changes in health status
- Provide assistance with personal hygiene, including bathing, grooming, and dressing
- Assist with mobility and physical therapy exercises
- Meet safety needs of patients and using equipment safely
- Perform light housekeeping tasks to maintain a clean and safe environment
- Offer companionship and emotional support to clients
- Document care provided on patient's electronic health record
Qualifications:
- Previous experience in home health care, nursing homes, or long-term care facilities is preferred
- Up to date credentials and health exams
- Knowledge of personal care, skin care, proper use of durable medical equipment, patient care, and hospice care practices
- Ability to follow care plans and communicate effectively with clients and healthcare providers
- Proficiency in taking vital signs and providing basic medical assistance
- Computer proficiency and experience with Wellsky/Kinnser a plus as electronic charting is required
- Compassionate attitude and dedication to enhancing the quality of life for clients The candidate must be willing to drive and have good time management skills in order to be efficient field staff.
