Job Description

Overview Join our dedicated team as a Home Health Aide and become a vital part of providing compassionate, personalized care to individuals in their homes. In this role, you will support clients with daily activities, monitor health status, and assist with medical needs, all while fostering independence and dignity. Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support using equipment such as Hoyer lifts. Monitor and record vital signs like blood pressure, pulse, respiration, and temperature to track health status accurately. Provide medication reminders and assist with medication administration following prescribed guidelines while maintaining HIPAA compliance. Support clients with specialized care needs such as Alzheimer's care, dementia care, memory care, hospice support, and working with individuals with developmental disabilities or disabilities. Document medical records and care notes using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems accurately and promptly. Assist with light housekeeping tasks including cleaning, laundry, meal preparation, and ensuring a safe living environment. Use medical terminology appropriately when documenting or communicating health-related information to healthcare teams. Employ patient observation skills to identify changes in condition or behavior that require escalation or intervention. Collaborate effectively with nursing staff and other healthcare professionals to ensure comprehensive patient care in home and community settings. Experience Prior experience in home health care or assisted living environments is highly preferred. Demonstrated experience working with seniors or individuals with disabilities in long-term care settings such as nursing homes or hospice environments. Familiarity with medical documentation practices and EHR/EMR systems is essential for maintaining accurate patient records. Knowledge of medical procedures including catheter care, CPR certification, vital signs measurement, and medication administration is required. Experience providing dementia or Alzheimer's care along with memory care strategies is highly valued. Proven ability to work independently while managing multiple clients effectively in a home or community setting. Strong understanding of HIPAA regulations to ensure client confidentiality at all times.
