In this vital role, you will provide personalized in-home care to individuals across various stages of life and health conditions, promoting independence, comfort, and well-being. Your expertise will support patients with diverse needs, including elderly individuals, those with disabilities, or those recovering from illness or surgery. This position offers a rewarding opportunity to deliver high-quality care while fostering trusting relationships with patients and their families. Responsibilities Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting, and mobility support Monitor and observe patient health status by recording vital signs and reporting changes to healthcare professionals Administer medications accurately following prescribed instructions and document medication administration in medical records Support patients with specialized care needs including Alzheimer's disease, dementia, hospice/end-of-life care, and memory care Operate Electronic Health Record (EHR) systems to maintain precise medical documentation and ensure compliance with home healthcare regulations Use assistive devices such as Hoyer lifts and perform catheter care safely and effectively Provide companionship and emotional support while assisting with light housekeeping tasks like cleaning, laundry, meal preparation, and ensuring a safe environment Skills Proven experience working with individuals with developmental disabilities, geriatrics, or in assisted living settings Strong knowledge of medical terminology, medication administration, vital signs monitoring, and basic life support (BLS) including CPR and first aid certifications Familiarity with home health agency operations and home & community care best practices Ability to work independently while adhering to HIPAA privacy standards and maintaining confidentiality of patient information Excellent patient observation skills to identify changes in condition or behavior promptly Experience working with Electronic Medical Records (EMR) systems or Electronic Health Records (EHR) systems for accurate documentation Physical ability to perform heavy lifting when necessary and assist with mobility using equipment like Hoyer lifts or other assistive devices Compassionate caregiving approach combined with strong communication skills to collaborate effectively with patients' families and healthcare teams Embark on a fulfilling career where your skills directly enhance the quality of life for those in need.
