This position offers an exciting opportunity to work closely with patients, supporting their daily needs and enhancing their quality of life. Your expertise will help create a safe, respectful, and nurturing environment for residents requiring ongoing assistance with their health and daily activities. Duties Assist residents with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support using equipment such as Hoyer lifts. Monitor patient health by observing vital signs and reporting changes promptly to nursing staff. Support medication administration following proper protocols and documentation standards while maintaining compliance with HIPAA regulations. Provide specialized care for residents with dementia, Alzheimer's disease, or other memory care needs, ensuring their safety and comfort. Assist with medical procedures such as catheter care, wound care, and hospice support as directed by healthcare professionals. Maintain accurate medical records using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems, ensuring thorough documentation of patient care activities. Support household tasks including cleaning, laundry, meal preparation, and maintaining a safe environment to promote overall well-being. Work collaboratively within a team to deliver home & community-based care that respects each individual's dignity and preferences. Engage in patient observation to identify potential health issues early and communicate findings effectively to the healthcare team. Follow all safety protocols during patient handling and use of equipment like the Hoyer lift or mobility aids. Assist with hospice care when needed, providing comfort and emotional support to patients and their families. Experience Prior experience working in nursing homes, assisted living facilities, or home health environments is highly preferred. Demonstrated ability to work with individuals with developmental disabilities or dementia-related conditions such as Alzheimer's disease. Knowledge of medical terminology and familiarity with medical documentation practices is essential. Certification in CPR (Cardiopulmonary Resuscitation) is required; additional certifications such as medication administration or catheter care are advantageous. Experience using EMR/EHR systems for documentation and patient record management is desirable. Strong skills in patient observation, caregiving, and home & community care are necessary to succeed in this role. Ability to handle physically demanding tasks safely, including lifting patients with equipment like Hoyer lifts or performing transfers.
