$25 - $31/hr

Position Summary:

Support older adults enrolled in a PACE program by providing hands-on personal care, companionship, and daily living assistance across the day center, participant homes, and medical appointments. You ll work closely with nurses and an interdisciplinary care team to help participants live safely, comfortably, and with dignity.

Key Responsibilities:

Provide direct personal care including bathing, toileting, feeding, transfers, mobility support, and companionship during medical visits and transportation. Assist with meal service, home delivery of meals and medical supplies, and light household or center-based tasks to support daily operations. Maintain clean, safe participant and program areas while following infection control and safety standards. Support team operations through documentation, basic clerical tasks, EHR data entry, and participation in trainings and meetings.

Qualifications:

Minimum 6 months experience as a Personal Care Aide; CNA candidates require at least 1 year of elderly care experience. High School Diploma or GED required; active AHA BLS certification required; valid California CNA license required for CNA roles. Strong communication skills and a compassionate, team-oriented approach to caring for older adults. Physically able to lift up to 50 lbs., assist with transfers, and perform repetitive movements such as bending, standing, and reaching.
