As a vital member of our caregiving team, you will assist clients with daily activities, monitor health conditions, and ensure their comfort and safety in a respectful, supportive environment. This role offers an opportunity to make a meaningful difference in the lives of those you serve while working in a dynamic and rewarding setting. Duties Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using tools like Hoyer lifts and other assistive devices. Support clients with specialized needs such as dementia care, Alzheimer's care, memory care, hospice care, and working with individuals with developmental disabilities or disabilities. Utilize Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems to document patient care activities thoroughly and accurately. Communicate effectively with healthcare teams about patient progress and any concerns related to medical records or treatment plans. Provide emotional support and companionship to promote mental well-being and foster a positive home environment. Follow all safety protocols during patient care activities, including CPR certification requirements and proper handling of catheters or other medical equipment. Qualifications Valid certification as a Certified Nursing Assistant or Home Health Aide from an accredited program; prior experience in home health or long-term care preferred. Demonstrated expertise in patient observation, assisting with ADLs, proper lifting mechanics, and hygiene support. Experience working with seniors in assisted living facilities or nursing homes is highly desirable. Knowledge of medical terminology and familiarity with EMR/EHR systems for accurate documentation. Strong understanding of HIPAA regulations to ensure patient confidentiality at all times. Ability to work independently while maintaining excellent communication within healthcare teams. CPR certification is required; additional certifications such as hospice care or dementia training are a plus.
