The ideal candidate will play a crucial role in enhancing the quality of life for our clients by assisting them with daily activities and promoting their independence. This position requires a strong commitment to care, patience, and the ability to work collaboratively with clients, families, and healthcare professionals. Responsibilities Assist clients with daily living activities such as personal hygiene, meal preparation, and medication management. Provide companionship and emotional support to clients, fostering a positive and encouraging environment. Implement individualized care plans tailored to each client's specific needs and preferences. Monitor client health and well-being through patient observation, reporting any changes or concerns to the appropriate personnel. Maintain a clean and safe living environment for clients, ensuring all areas are tidy and organized. Support clients in social activities and community engagement, promoting inclusion and participation. Collaborate with healthcare providers, family members, and other professionals involved in the client's care. Document client progress and any incidents accurately in accordance with company policies. Transportation; use company vehicles to pick up and drop off individuals to and from home for the daily activities. Experience Previous experience in social work, dementia care, hospice care, or long-term care is preferred but not required. Familiarity with working with people with disabilities is highly desirable. Basic knowledge of first aid practices is beneficial. Experience in home health settings is a plus. Strong communication skills and the ability to build rapport with clients are essential. A compassionate demeanor and a genuine interest in helping others are critical for success in this role.
