Location:
Individual's Homes/ Office Job Overview We are seeking a compassionate and reliable care provider/care coordinator to support individuals with intellectual and developmental disabilities (IDD) who live independently in the community. This role focuses on promoting independence, supporting daily living skills, and helping ensure individuals receive the support and services they need to thrive. The support/care coordinator will also serve as a consistent point of contact, helping maintain communication between the individual, families, and members of the support team. Responsibilities Provide person-centered support in the individual's home and community. Assist with daily living and independent living skills (organization, scheduling, household tasks). Support participation in community activities and appointments. Monitor health, safety, and overall well-being. Maintain communication with supervisors and support teams regarding updates or concerns. Assist with coordinating appointments and community resources when needed. Complete required service documentation accurately and on time. Qualifications High school diploma or GED required. Experience working with individuals with intellectual and developmental disabilities (IDD) preferred. Experience in human services, caregiving, healthcare, or community support roles preferred. Strong communication and organizational skills. Ability to work independently and be dependable. Valid driver's license and reliable transportation preferred.
