Job Description

Overview We are seeking compassionate and dedicated Personal Care Assistants/Homemakers to provide exceptional in-home and community-based care for individuals with diverse needs. This role involves supporting clients with daily activities, health monitoring, and personal care in a respectful and professional manner. Ideal candidates will have a strong desire to improve quality of life for those they serve, with experience in senior care, disability support, or home health services. Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Provide medication administration and monitor vital signs in accordance with care plans Support clients with Alzheimer's, dementia, or memory care needs through specialized techniques and patience Prepare nutritious meals tailored to dietary requirements and assist with meal planning and preparation Perform light housekeeping duties such as cleaning, laundry, and maintaining a safe living environment Use patient observation skills to monitor health status and report any concerns promptly Document care activities accurately while respecting HIPAA regulations Support clients in community settings or residential facilities, including assisted living or group homes Collaborate with healthcare professionals to implement individualized care plans Engage clients in social activities to promote mental well-being and social interaction Experience Prior experience in home & community care, assisted living, nursing homes, or long-term care environments preferred Knowledge of patient monitoring, vital signs, medication administration, and dementia or Alzheimer's care is highly desirable Experience working with individuals with developmental disabilities, autism, or other special needs is a plus Certifications such as CPR and First Aid are preferred but not mandatory; training can be provided if necessary Familiarity with HIPAA regulations ensures confidentiality and privacy of client information Previous roles involving heavy lifting or use of mobility aids like Hoyer lifts are beneficial Experience working with diverse populations including seniors, individuals with disabilities, or those requiring hospice or palliative care is valued This role requires compassionate individuals committed to providing high-quality personal care. Candidates should be dependable, attentive to detail, patient-focused, and capable of working independently while adhering to safety standards.
