Responsibilities Provide attentive assistance with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using tools such as Hoyer lifts when necessary. Monitor patient health by observing vital signs, administering medication, and documenting medical records accurately within Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems. Assist with specialized care needs such as catheter care, hospice support, Alzheimer's and dementia care, and memory care routines. Support residents in home and community settings by helping with meal preparation, cooking, laundry, cleaning, and maintaining a safe living environment. Maintain compliance with HIPAA regulations to ensure confidentiality of all medical and personal information. Use medical terminology appropriately when recording observations or communicating with healthcare professionals. Demonstrate proficiency in working with individuals with developmental disabilities or other special needs while fostering independence and dignity. Skills Strong patient observation skills to detect changes in health status promptly. Experience in assisted living facilities, nursing homes, or home health environments is highly desirable. Familiarity with EMR/EHR systems for accurate documentation of patient care activities. Knowledge of ADLs assistance techniques including mobility aids like Hoyer lifts and vital signs measurement. Ability to perform medication administration safely following prescribed protocols. Competence in dementia and Alzheimer's care strategies to support memory care residents effectively. CPR certification and understanding of medical terminology for clear communication. Experience with catheter care, hospice support, and working within long-term care settings enhances your suitability for this role.
