Here's how you'll make an impact: Assess psychosocial and financial needs of the patient to identify and implement a plan, using community resources and referrals, to assist the patient in maintaining self at home, counseling and end-of-life decisions. The Social Worker may assist with placement if patient unable to be managed in the home setting.Minimum Qualifications:
Graduation from a masters degree social work program accredited by the Council on Social Work Education Valid driver's license in good standing Minimum of 3 years of experience as a social worker in a health care setting Must have own transportation and be reliable Flexible availability Good organization and communication skills Ability to follow and carry out instructions Experience with electronic documentation/point of care systems a plus At Masters In Home Care, we believe in nurturing our team.
