This vital role involves delivering personalized care, ensuring safety, and promoting independence for clients with diverse needs, including elderly individuals, those with disabilities, or patients requiring specialized medical attention. The ideal candidate will be energetic, detail-oriented, and committed to enhancing the quality of life for those they serve. Duties Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and toileting to promote comfort and dignity. Monitor patient health by observing vital signs and reporting any changes or concerns promptly. Support medication administration following prescribed medical protocols while maintaining accurate medical documentation. Utilize Electronic Health Record (EHR) and Electronic Medical Record (EMR) systems to document care activities and patient progress efficiently. Provide specialized care including Alzheimer's, dementia, memory care, hospice support, and working with individuals with developmental disabilities or disabilities. Operate medical equipment such as Hoyer lifts, catheters, and other assistive devices safely and effectively. Maintain a clean and safe environment by assisting with light housekeeping tasks like laundry, cleaning, meal preparation, and ensuring infection control standards are met in compliance with HIPAA regulations. Support patients during mobility activities using tools like Hoyer lifts or transfer belts to prevent falls or injuries. Engage in patient observation to detect changes in condition or behavior that may require medical attention or intervention. Collaborate closely with healthcare teams to ensure comprehensive patient care in home or community settings. Skills Strong knowledge of medical terminology and caregiving best practices across various settings including assisted living, nursing homes, and home health environments. Proven experience working with seniors, individuals with disabilities, dementia or Alzheimer's disease, and those requiring memory care or hospice services. Excellent communication skills to interact empathetically with clients and coordinate effectively with healthcare professionals. Ability to operate EMR/EHR systems accurately for documentation purposes. Competence in taking vital signs such as blood pressure, pulse, temperature, and respiration rates. Knowledge of medication administration protocols adhering to safety standards and HIPAA privacy rules. Physical stamina to perform caregiving tasks involving lifting, transferring, cleaning, cooking, and other household chores. CPR certification and training in catheter care or other specialized medical procedures preferred but not mandatory; training provided if needed. Experience working in home & community care environments with a focus on patient safety and comfort. Demonstrated ability to work independently while following detailed care plans in live-in employment or shift-based roles.
