In this vital role, you will provide compassionate assistance with daily activities, monitor health conditions, and ensure a safe, comfortable environment for clients in their homes or community settings. Your energy and attention to detail will help promote independence, dignity, and well-being for those you serve. This position offers an engaging opportunity to work closely with diverse populations, including seniors, individuals with disabilities, and those receiving hospice or dementia care. Responsibilities Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting, and mobility support using equipment like Hoyer lifts when necessary Monitor and record vital signs including blood pressure, pulse, temperature, and respiration to track health status accurately Provide companionship and emotional support to reduce feelings of loneliness and promote mental well-being Perform light housekeeping tasks such as cleaning, laundry, meal preparation, and ensuring a safe living environment for the patient Document time and attendance using Electronic Visit Verification software and apps. Assist with specialized care needs including catheter care, dementia or Alzheimer's care, hospice support, and working with individuals with developmental disabilities or disabilities Skills Proven experience in home health or long-term care environments with knowledge of medical terminology and documentation practices Strong patient observation skills to identify changes in condition or behavior promptly Familiarity with EVV systems for efficient time keeping and communication Ability to perform CPR and handle emergency situations confidently Experience working with individuals with dementia, Alzheimer's disease, or other memory impairments in memory or hospice care settings Competence in assisting with mobility devices such as Hoyer lifts and performing vital signs assessments Compassionate caregiving attitude combined with excellent communication skills to build trustful relationships This role is ideal for motivated individuals passionate about providing high-quality home care.
