This energetic position offers the opportunity to make a meaningful difference in the lives of seniors, individuals with disabilities, and those recovering from illness or managing chronic conditions. You will provide essential support with daily activities, health monitoring, and emotional encouragement, fostering independence and dignity for your clients. Duties Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting, and mobility support using equipment like Hoyer lifts when necessary Monitor and record vital signs including blood pressure, pulse, temperature, and respiration to ensure client health status is maintained Administer medications accurately following medical documentation protocols while respecting HIPAA privacy standards Support clients with meal preparation and nutritional needs, ensuring dietary restrictions are followed Conduct patient observation to identify changes in condition or behavior and communicate findings promptly to healthcare professionals Provide companionship and emotional support to enhance mental well-being and reduce feelings of isolation Maintain clean and safe living environments through light housekeeping tasks such as laundry, cleaning, and organizing to promote overall health and safety Utilize Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems for accurate documentation of care activities and medical records Assist with specialized care needs including dementia care, Alzheimer's care, hospice support, catheter care, and working with individuals with developmental disabilities or disabilities Support safe transfers and mobility using assistive devices like Hoyer lifts or transfer belts while adhering to safety protocols Experience Prior experience in home health care or assisted living environments preferred but not required; training provided for motivated candidates Demonstrated ability to work effectively with seniors and individuals with disabilities or chronic health conditions such as dementia or memory care needs Knowledge of medical terminology and basic medical procedures including vital signs measurement and medication administration Experience working with EMR/EHR systems for accurate record keeping is advantageous Long-term care experience in nursing homes or hospice settings is a plus but not mandatory Certification in CPR is required; additional certifications such as hospice care training or dementia care are beneficial Strong communication skills coupled with a compassionate attitude are essential for success in this role Join us to provide compassionate home health services that empower clients to live safely and comfortably in their own homes.
