Day Program Instructor (Full-Time/Non Exempt)

Monday-Friday, 7 AM-3 PM We are seeking one full-time Day Program Instructor to join our team. In this role, you will assist in implementing person-centered supports for individuals in our Day Services Program. You will help with daily activities, personal care, and community involvement, ensuring that quality care is provided in line with Federal, State, and local standards. Position Summary This position will serve as an integral member of the Day Services Team and will assist with the day-to-day activities of the program. Assist in the implementation of person centered supports in the habilitation process including personal care, daily activities and community-based activities focusing on the participants choices. Ensures that quality of care, services and training are provided to individuals in accordance with Federal, State, and local standards.
Key Responsibilities:
Assist individuals with their Individual Support Plans (ISPs) and activity schedules. Support the development of independent living and social skills through person-centered planning. Maintain a safe and clean environment, assisting with transportation for community and vocational activities. Document participant progress, health, and safety, and contribute to ISP meetings. Monitor participants for changes in health or behavior and report concerns as needed. Ensure adherence to all agency policies, as well as Federal, State, and Local regulations.
Required Skills and Qualifications:
Compassionate, patient, and nurturing demeanor. Ability to work collaboratively and adapt to change. Strong written and verbal communication skills in English. Valid Driver's License with a clean driving record for the past three years. Willingness to complete required training and competencies.
PHP/FPDC
is an intermediate care facility for the intellectually and developmentally disabled. We care for individuals 24/7 in a set of four (4) homes that hold up to 16 individuals. We also have a Day Program the clients attend Monday - Friday.
