Duties Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor and record vital signs such as blood pressure, pulse, temperature, and respiration using EMR (Electronic Medical Record) systems Support medication administration following medical documentation protocols and HIPAA regulations to ensure patient privacy and safety Provide specialized care for patients with dementia, Alzheimer's disease, or other memory impairments, including memory care techniques and Alzheimer's-specific activities Operate equipment like Hoyer lifts and perform catheter care or hospice-related tasks with sensitivity and professionalism Assist with meal preparation, feeding, light cleaning, laundry, and maintaining a safe home environment Document patient progress accurately in medical records and communicate effectively with healthcare teams regarding patient status or changes Experience Prior experience in home health care, assisted living facilities, nursing homes or long-term care environments preferred Familiarity with working with people with developmental disabilities or disabilities requiring specialized support Knowledge of medical terminology, CPR certification, and skills in medical documentation are highly valued Experience using EMR or EHR (Electronic Health Record) systems to record patient information efficiently Demonstrated ability to work compassionately with seniors or individuals needing hospice or dementia care Proven ability to handle tasks such as cleaning, cooking, laundry, and maintaining hygiene standards within a home care setting Strong interpersonal skills to connect empathetically with patients while adhering to HIPAA guidelines Embark on a rewarding career that empowers you to provide essential care in a home setting.
