Job Description
Job Overview Join our dedicated healthcare team as a Home Health Aide, where you will play a vital role in providing compassionate, personalized care to individuals in their homes. As a Home Health Aide, you will work closely with clients, families, and healthcare professionals to deliver high-quality care that promotes independence and enhances quality of life. Responsibilities Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support using tools such as Hoyer lifts and other assistive devices. Monitor patient health by observing vital signs like blood pressure, pulse, temperature, and respiration; accurately document findings in Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems. Support medication administration under supervision, ensuring proper documentation while adhering to HIPAA regulations for patient privacy and confidentiality. Provide specialized care for individuals with dementia or Alzheimer's disease through memory care techniques and tailored activities. Assist with medical procedures such as catheter care, wound care, and hospice support as directed by healthcare professionals. Prepare nutritious meals and assist with feeding while considering dietary restrictions; perform light cleaning and laundry to maintain a safe and hygienic environment. Document all patient interactions thoroughly in medical records, ensuring accuracy for ongoing care coordination. Collaborate with nursing staff and other healthcare providers to deliver comprehensive home health services that meet each patient's unique needs. Experience Prior experience working in assisted living facilities, nursing homes, or home health environments is highly preferred. Demonstrated ability to work effectively with people with developmental disabilities or those requiring memory care such as Alzheimer's or dementia care. Familiarity with medical terminology, medical documentation practices, and EHR/EMR systems is essential. Certification in CPR is required; additional certifications such as medication administration, catheter care, hospice care support, or Hoyer lift training are advantageous. Experience working with patients who have chronic conditions or are receiving long-term care is highly valued. Proven ability to handle sensitive situations with compassion while maintaining professional boundaries and confidentiality under HIPAA guidelines.
