Duties Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support using patient transfer methods and equipment such as Hoyer lifts. Monitor and document patient health status by observing vital signs, recognizing changes in condition, and reporting findings accurately using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems. Administer medications following prescribed protocols and ensure proper medication documentation in compliance with HIPAA and home health regulations. Support patients with medical procedures such as catheter care, wound dressing changes, and basic first aid or CPR when necessary. Provide companionship and emotional support to clients with dementia or Alzheimer's disease, ensuring a safe environment for memory care and geriatric patients. Assist with meal preparation, light cleaning, laundry, and maintaining a clean and safe living environment for clients. Communicate effectively with clients' families and healthcare teams to coordinate care plans and update medical records accurately. Qualifications Proven experience working as a Home Health Aide or Caregiver in assisted living facilities, nursing homes, hospice care, or home healthcare settings. Knowledge of medical terminology, patient observation techniques, and home health care assistance standards. Experience working with individuals with developmental disabilities, dementia, Alzheimer's disease, or other cognitive impairments. Ability to perform heavy lifting safely and operate patient transfer equipment such as Hoyer lifts. Certification in First Aid and Basic Life Support (BLS), including CPR training; additional certifications in hospice or end-of-life care are a plus. Familiarity with EMR/EHR systems for medical documentation; understanding of home health agency regulations is preferred. Strong communication skills for client interaction and family updates while maintaining patient confidentiality per HIPAA guidelines. Ability to work independently in live-in employment settings or flexible schedules that meet client needs.
