This position offers an exciting opportunity to make a meaningful difference in the lives of patients by assisting with daily activities, ensuring safety, and promoting independence. As a Home Health Aide, you will work closely with clients and their families to deliver respectful, attentive care that fosters comfort and well-being. Your dedication and skills will help create a supportive environment for those navigating health challenges or aging in place. Responsibilities Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support. Monitor and observe patient conditions, recording vital signs such as blood pressure, pulse, and temperature accurately. Support medication administration under supervision, ensuring proper timing and documentation in electronic health record (EHR) systems. Provide specialized care for individuals with dementia or Alzheimer's disease, including memory care techniques and safety measures. Assist with medical procedures such as catheter care, Hoyer lift transfers, and hospice care support when needed. Maintain clean and safe environments by performing light housekeeping tasks like laundry, cleaning, meal preparation, and ensuring infection control protocols are followed. Document all patient interactions and medical records in compliance with HIPAA regulations while utilizing EMR (Electronic Medical Records) systems for accurate record-keeping. Collaborate effectively with nursing staff and other healthcare professionals to ensure comprehensive patient care plans are implemented smoothly. Requirements Proven experience working in assisted living facilities, nursing homes, or home health settings with a focus on senior care or long-term care. Strong knowledge of medical terminology, patient observation techniques, and caregiving best practices. Valid CPR certification; additional certifications such as hospice care or dementia training are a plus. Ability to operate medical equipment including Hoyer lifts and EHR/EMR systems efficiently. Experience working with individuals with developmental disabilities or disabilities requiring specialized support. Excellent communication skills to interact empathetically with patients and their families while maintaining confidentiality per HIPAA guidelines. Flexibility to work live-in shifts or part-time schedules based on client needs; reliable transportation is preferred.
