Job Overview Join our dedicated team as a Home Health Aide & Caregiver, where your compassionate care and attention to detail will make a meaningful difference in the lives of our clients. In this vital role, you will provide personalized support to individuals in their homes or assisted living facilities, helping them maintain independence, dignity, and comfort. Your energetic and proactive approach will ensure that each client receives the highest quality of care, fostering a safe and nurturing environment. This position offers the opportunity to work closely with diverse populations, including seniors, individuals with developmental disabilities, and those requiring specialized dementia or Alzheimer's care. Responsibilities Provide compassionate assistance with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support. Monitor and observe patients' conditions regularly, accurately documenting vital signs such as blood pressure, pulse, temperature, and respiration using EMR (Electronic Medical Records) or EHR (Electronic Health Records) systems. Assist with medication administration following prescribed schedules and proper medical documentation protocols while adhering to HIPAA regulations to protect patient privacy. Support clients with meal preparation and nutritional needs, ensuring dietary restrictions are followed precisely. Utilize specialized equipment such as Hoyer lifts for safe patient transfers and perform catheter care when necessary. Engage in dementia and memory care activities tailored to individual needs, including Alzheimer's care strategies and memory-enhancing exercises. Maintain a clean and safe environment by assisting with light housekeeping tasks such as laundry and general cleaning in accordance with health standards. Collaborate with healthcare professionals by updating medical records accurately and participating in care planning meetings. Provide emotional support and companionship to foster a positive atmosphere for clients with hospice or end-of-life needs. Demonstrate proficiency in working with EMR/EHR systems for medical documentation and patient records management. Follow all safety protocols including CPR certification requirements and proper use of mobility aids like Hoyer lifts. Support clients in community settings or during live-in employment arrangements, ensuring consistent quality of care across different environments. Qualifications Proven experience as a Home Health Aide or Caregiver in settings such as assisted living facilities, nursing homes, or home health environments. Strong knowledge of patient observation techniques, vital signs monitoring, medication administration, and medical terminology. Experience working with individuals with developmental disabilities or dementia-related conditions such as Alzheimer's disease is highly preferred. Certification in CPR (Cardiopulmonary Resuscitation) is required; additional certifications like hospice care training or catheter care are advantageous. Familiarity with EMR/EHR systems for accurate medical documentation; understanding of HIPAA compliance is essential. Ability to assist with ADLs efficiently while maintaining a respectful and empathetic approach toward clients' dignity. Capable of working independently or as part of a team within home & community care settings; flexibility for live-in employment is a plus. Excellent communication skills to effectively coordinate with healthcare teams and family members. Physical ability to stand, walk, lift patients safely using appropriate equipment, and perform routine caregiving tasks over extended periods.
