Job Description
Job Overview As a Home Health Aide, you will play a vital role in enhancing the quality of life for individuals in need of assistance within their homes. Your compassionate care and support will empower clients to maintain their independence while ensuring their safety and comfort. You'll engage with patients, assist with daily living activities, and provide essential companionship, making a meaningful difference in their lives. What you'll do Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and meal preparation. Monitor and document vital signs, ensuring any changes are reported to the healthcare team. Provide medication administration as prescribed and assist with catheter care when necessary. Support clients with mobility using equipment like Hoyer lifts and ensure safe transfers. Maintain a clean and safe environment by performing light housekeeping tasks including cleaning and laundry. Engage clients in meaningful conversation and activities to promote mental stimulation and emotional well-being. Uphold patient confidentiality by adhering to HIPAA regulations while managing medical records and documentation. Collaborate with family members and healthcare professionals to develop personalized care plans that meet each client's unique needs. Skills Strong understanding of medical terminology and experience with Electronic Medical Records (EMR) or Electronic Health Records (EHR) systems. Proven ability to provide compassionate caregiving for seniors, individuals with disabilities, or those requiring Alzheimer's or dementia care. Knowledge of hospice care practices and long-term care experience is highly valued. Certification in CPR is required; additional training in patient observation is preferred. Excellent communication skills to effectively interact with clients, families, and healthcare teams. Ability to perform cleaning tasks efficiently while maintaining a positive attitude. Experience working with people who have developmental disabilities is an asset; familiarity with home and community care is beneficial.
