Job Description

Job Summary Join our compassionate healthcare team as a Home Health Aide (HHA), where you will play a vital role in delivering personalized, high-quality care to individuals in their homes. This energetic position offers the opportunity to make a meaningful difference in patients' lives by assisting with daily activities, providing emotional support, and ensuring their safety and comfort. You will work closely with clients, families, and healthcare professionals to promote independence and well-being in a warm, supportive environment. Duties Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using tools like Hoyer lifts when necessary Monitor patient health by taking vital signs such as blood pressure, pulse, temperature, and respiration; document medical records accurately using Electronic Health Record (EHR) systems Support medication administration following proper protocols while maintaining HIPAA compliance to protect patient privacy Provide specialized care for individuals with dementia or Alzheimer's disease, including memory care strategies and behavioral management Assist with medical procedures such as catheter care, wound dressing changes, and hospice-related comfort measures Prepare nutritious meals and assist with feeding to meet dietary needs and preferences Maintain a clean and safe environment by performing light housekeeping tasks like laundry, cleaning, and organizing patient spaces Requirements Proven experience in home health or long-term care environments; nursing home experience is highly valued Strong knowledge of medical terminology, patient observation techniques, and documentation practices within EMR/EHR systems Certification in CPR (Cardiopulmonary Resuscitation) is required; additional certifications such as medication administration or hospice care are a plus Ability to work independently and as part of a team while managing multiple patient needs effectively Experience working with individuals with developmental disabilities or disabilities requiring specialized attention Familiarity with assistive devices such as Hoyer lifts and mobility aids Compassionate attitude with excellent communication skills to connect with patients and their families Flexibility to work live-in shifts or flexible hours based on client needs Join us in providing exceptional home health care that empowers individuals to live safely and comfortably at home.
