Job Description

Job Overview We are seeking a compassionate and dedicated Home Health Aide to provide exceptional in-home care to bedbound senior requiring assistance with daily activities. The ideal candidate will possess a strong commitment to patient well-being, demonstrate excellent caregiving skills, and have experience working with diverse populations, including seniors, individuals with disabilities, and those receiving hospice or dementia care. As a vital member of our healthcare team, you will support clients in maintaining their independence and quality of life within the comfort of their homes. Duties Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support. Monitor and observe patients' health status, recording vital signs and reporting any changes to healthcare professionals. Administer medications accurately following prescribed instructions while maintaining compliance with HIPAA regulations. Support clients with medical procedures such as catheter care, wound care, and using assistive devices like Hoyer lifts. Provide companionship and emotional support, especially for individuals with dementia or Alzheimer's disease. Prepare nutritious meals tailored to dietary restrictions and assist with meal planning and preparation. Perform light housekeeping tasks such as cleaning, laundry, and maintaining a safe environment for clients. Document medical records accurately using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems to ensure proper communication among healthcare providers. Skills Proven experience in home health care, long-term care, or assisted living environments. Strong knowledge of medical terminology and caregiving best practices for seniors and individuals with disabilities. Ability to work effectively with individuals experiencing memory loss or cognitive impairments such as dementia or Alzheimer's disease. Proficiency in using EMR/EHR systems for documentation and medical records management. CPR certification and knowledge of emergency response procedures are required. Experience working with hospice patients and providing compassionate end-of-life care. Familiarity with medical procedures including vital signs measurement, medication administration, catheter care, and hospice protocols. Ability to operate mobility aids such as Hoyer lifts safely and effectively. Excellent communication skills to collaborate with clients' families and healthcare teams professionally.
