  • Full-time We are seeking a compassionate and experienced Licensed Social Worker (LSW) to join our Home Care team!
This role is responsible for supporting patients and families through assessments, care coordination, advocacy, education, and community resource management to promote overall well-being and quality of life. The ideal candidate is organized, detail-oriented, and passionate about providing exceptional patient-centered support in a collaborative and caring environment.
Minimum Requirements:
Current Florida Licensed Social Worker (LSW) credentials required Knowledge of laws, regulations, and guidelines related to Senior Living and Home Health Minimum one year of experience in a healthcare setting preferred
Knowledge, Skills & Abilities:
Strong organizational and communication skills Detail-oriented with the ability to maintain accuracy daily Excellent interpersonal skills with a professional and compassionate approach Proficient in Microsoft Office programs including Excel, Outlook, and Word Ability to learn and use proprietary software as needed Responsibilities Develop, implement, and oversee comprehensive programs tailored to meet the needs of individuals with developmental disabilities. Manage day-to-day program operations, ensuring compliance with all regulatory standards and organizational policies. Supervise staff and volunteers, providing guidance, training, and performance evaluations to foster a motivated team environment. Build and maintain trusting relationships with clients, families, and community stakeholders to promote collaborative support networks. Coordinate behavioral health services and behavioral management plans to support clients' emotional well-being. Oversee administrative functions including budgeting, record keeping, documentation, and reporting to ensure operational efficiency. Lead program development initiatives aimed at expanding services and enhancing quality outcomes for participants. Experience Proven management experience in social services or related fields, with a focus on program development and supervision. Hands-on experience working with people with developmental disabilities or behavioral health needs. Strong relationship management skills to effectively collaborate with clients, families, community agencies, and internal teams. Demonstrated ability to handle administrative responsibilities such as budgeting, documentation, and compliance requirements. Background in social work or related behavioral health disciplines is highly desirable. Prior experience supervising staff or volunteers in a fast-paced environment is preferred. Knowledge of program management principles and best practices for supporting individuals with diverse needs.
