We provide caring and reliable assistance to individuals with disabilities who would like support getting out into the community, participating in activities of their choice, and working toward their personal goals.

Duties Assist residents with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support. Monitor residents' health status by observing vital signs, patient behavior, and overall well-being. Administer medications following prescribed care plans while adhering to HIPAA regulations for confidentiality. Support residents during meal preparation and ensure proper nutrition through cooking and feeding assistance. Maintain a clean, safe, and organized living environment by performing light cleaning and sanitation tasks regularly. Use specialized equipment such as Hoyer lifts or other assistive devices to ensure safe patient transfers and mobility. Implement individualized care plans tailored to each resident's needs, including dementia, Alzheimer's, or autism-related care. Engage residents in social activities and community outings to promote socialization and mental stimulation. Document observations accurately in compliance with healthcare standards and report any changes in condition promptly. Requirements Proven experience working in group homes, assisted living facilities, nursing homes, or home health environments. Knowledge of patient care techniques including vital signs monitoring, CPR certification, first aid, and medication administration. Familiarity with HIPAA regulations and confidentiality standards related to patient information. Ability to perform heavy lifting safely and operate assistive devices such as Hoyer lifts or wheelchairs. Experience working with individuals with developmental disabilities, dementia, Alzheimer's disease, or autism spectrum disorders. Strong communication skills coupled with patience and empathy for diverse resident needs. Prior experience in long-term care settings or hospice care is advantageous but not mandatory. Ability to follow detailed care plans while demonstrating flexibility in dynamic environments.
